These two forms must be filled out and returned by all science party members:

• Next of Kin / Security ID Form     

• Medical History and HIPAA release Form [MS Word]

Chief Scientists of scheduled legs should read the instructions and fill out these forms:

• Shipboard Equipment Form

• Science Party List

If foreign clearance is required, please complete requests as necessary:

• Foreign Clearance request

and, if hazardous materials or chemicals are to be used, this one too:

• HAZMAT inventory & control Form
