Description:
IESX Error attempting to add data storage location (DSL) to a user:

  Data storage location root handling failed
  Add data storage location root failed
  Device record not found

Solution:
The error stems from:

1. A disk listed in Administration > Configuration > Workstation Configuration 
    is no longer a valid disk.

OR

2. A disk assigned to a user in Administration > Configuration > Data Storage 
    Locations is no longer listed in Workstation Configuration.

Remove the disk from the list. Retry adding the DSL.
