'GCPTOPIN - wrong well name on ptop card' error when loading well data.

Description:
Set up a user defined format definition for loading well data from an ASCII file.  

Solution:
The problem was that the format definition specified the incorrect card columns
for reading in the well name.  Adjusted the card columns, and the well file
loaded without errors.
