What are the required fields to ASCII import interpretation?

Description:
In exporting data in a multi vendor environment, it is useful to know the
minimum required fields IESX will need in the output file in order to later
import the data.

Solution:
2d ASCII interpretation data:
 
2d Horizon and contact data:

Note 1.  When using the override option, the survey, line name for 2d and
                horizon may be selected.  They will not need to be in the file.

Note 2.  The z-value must be loaded as float, not integer.
